Searching multiple criteria

I am trying to use 3 criteria but I'm not seeing the other selections (drop down selections) in the form variables on the result page. The criteria utilizes 2 drop downs plus the date range. The following is for a drop down. Can anyone tell me why the search criteria for the pull downs is not being passed along with the date range search?
