I am currently dealling with some client database issues as well, and I think if more information was out there it could be helpful. The "worked great for years" thing is something I'm familiar with too. Recently one of my clients sites appeared to have a core OS date/time stamp issue (networed time server did something bad at spring forward). I believe this may have messed with the safe-write system... which then caused a domino affect on some of the heavily used .db's that it was protecting. I was able to isolate the time when the core OS date/time clock was munged.. the .db issues started happing about an hour or so later.
